One of the principal aims of the Primary & Community Mental Health Transformation Programme is to create an environment where the Statutory and Voluntary sectors can work more effectively together.

The voluntary sector provides an essential role in helping to meet social and wellbeing needs and Sheffield has a strong network of specialist local and city wide organisations providing essential care and support. The Transformation Programme has made funds available to resource new and innovative work through the award of grants (the Community Connection Fund) and also through the creation of a new service, the Community Connectors.

Community Connectors

The Community Connectors will eventually be based in the 4 Primary Care Networks that pilot the scheme (post Covid-19). However, despite being home-based currently, they are still helping people to access wider community support by removing the barriers that might otherwise prevent them from engaging.

Our  Connectors work with the Primary Care Networks (PCN's) to ensure that people with serious mental illness get the most appropriate local support and help individuals to more effectively engage and integrate with their local community.
