One of the principal aims of the Primary & Community Mental Health Transformation Programme was to create an environment where the Statutory and voluntary sectors could work more effectively together.

The voluntary sector provides an essential role in helping to meet social and wellbeing needs and Sheffield has a strong network of specialist local and city wide organisations providing essential care and support. The programme will make funds available to resource new and innovative work through the award of grants (the Community Connection Fund) and also with through the creation of a new service, the Community Connectors.

Community Connectors

The community connectors will be based in the 4 Primary Care Networks that will pilot the scheme and help people to access wider community support by removing the barriers that might otherwise prevent them engaging.

The Connectors will work with the PCN's to ensure that people with serious mental illness get the most appropriate local support and help individuals more effectively engage and integrate with their local community.

Further details about the service will be made shortly - watch this space
